REQUIREMENTS:

1. Certificate as Registered Nurse
2. Certificate as Registered Midwife
3. Certificate as B.Sc Nursing
4. Current Practising Licence
5. Local Government Identification
6. Birth Certificate/Age Declaration (Not more than 45yrs)
7. First School Leaving Certificate
8. WAEC/NECO
9. NYSC Certificate (where applicable)
