NDUTH Documentation Requirements for newly employed

All newly employed are requested to go  for documentation  with a personnel file composed of the following:

1.  Personnel application letter
2. Personnel curriculum vitae (cv)
3. Appointment letter
4. Educational qualification(s)
5. Birth certificate or age declaration
6. Local govt. Identification letter
7. Current practicing license (where applicable)
8. acceptance letter
9. Medical certificate of fitness
10. Personnel record of service
11. Two passport size photograph
